The Community Health Center of the New River Valley (CHCNRV) – Christiansburg site – has been recognized by the National Committee for Quality Assurance and has received the NCQA Patient-Centered Medical Home Recognition, Level 3, for using evidence-based, patient-centered processes that focus on highly coordinated care and long‐term, participative relationships.
The medical home model is a method developed to improve healthcare by transforming how primary care is organized and delivered. The model combines teamwork, technology and accessibility to improve care, improve patients’ experience of care and reduce costs. Medical homes foster ongoing partnerships between patients and their practitioners with an orientation toward the whole person. Each patient’s care is overseen by care teams that coordinate treatment across all elements of the health system. Research indicates that medical homes can lead to higher quality and lower costs, and can improve patient and provider experiences.
To earn recognition, which is valid for three years, CHCNRV demonstrated the ability to meet the program’s elements which embody characteristics of the medical home. NCQA standards align with the principles of the Patient-Centered Medical Home established with the American College of Physicians, the American Academy of Family Physicians, the American Academy of Pediatrics and the American Osteopathic Association. With the commitment from staff and physicians, CHCNRV has taken another step toward ensuring their patients receive the highest quality of care.
