that the orthodontic practice of R. L. Turner, D.D.S., Ltd., owned and operated by R. L. Turner, D.D.S., with offices at 800 S. Main Street, Blacksburg, Virginia 24060 and 301 Radford Street, Christiansburg, Virginia 24073, will cease doing business and will no longer see patients, effective November 15, 2019. At the written request of any patient of the practice or an authorized representative of such patient, the practice’s records (or copies thereof) with respect to such patient will be sent by the practice, within a reasonable time after such request, to another licensed orthodontist or to such patient or authorized patient representative. The patient shall be responsible for the actual costs of copies and deliveries so requested.
