OF FEE-FOR-SERVICE REMOTE PATIENT CARE SERVICES

CopilotIQ will permanently end its fee-for-service remote patient monitoring (RPM) services on October 15, 2026.

This affects only patients receiving RPM on a fee-for-service basis; patients enrolled through a health plan are unaffected and will continue without interruption. Affected patients should arrange alternative care before that date.

For a copy of your records, submit a written request to the address below; to transfer records to another provider, submit a written authorization including the provider name and address.
